There is no single national format
What a registration card has to carry comes from state and local rules and from your own licensing conditions, so what is expected in your district is not necessarily what is expected in the next one. The fields here are the ones a card almost always includes, which makes this a sound starting point rather than a compliance guarantee.
Check it against what your local authority actually asks for before you print a pad. If a district requires something we have not thought of, the card is worth more with that field than without it.
About the identity document
A government photo ID that also shows an address is what is usually wanted — Aadhaar, a driving licence, a voter ID or a passport. A PAN card is commonly refused, because it is a photo ID with no address on it.
Record what you were shown rather than what the guest says they have. The card is the record that the check happened.
A foreign guest needs a Form C as well
This card is your own record and it is not a substitute for reporting a foreign national's arrival to the Bureau of Immigration. That is a separate obligation with its own deadline and it is filed on a government portal.
If you take foreign guests, treat the two as one routine: the card at the desk, the report within the day.
Your details are kept, the guest's are not
Your property name, address and phone are remembered in this browser so you are not retyping them for every card. They are yours and they are not personal data about anybody else.
Everything about the guest — name, address, identity document, contact — is held only while the tab is open. It is never sent to us and never written to storage. A front desk is usually a shared machine, and a guest's ID number should not still be there when the next shift sits down.
